How do I order new insurance cards?
To order new prescription cards, contact 844-757-0452.
To order new medical cards, contact BCBSNE at 844-664-9371. Fire Trust members can contact Imagine360 at 800-784-8153.
To order new dental cards, contact Ameritas at 800-487-5553. To contact new vision cards, contact Ameritas at 800-877-7195.
I lost my HSA Bank card, how do I get a new one?
You can report your card lost or stolen through your member account at HSABank.com or you can call HSA Bank directly at 800-357-6246.
I have had a benefit life event, what do I do now?
Contact benefits@cityofomaha.org within 30 days of the life event and provide documentation of the event such as marriage certificate, divorce decree, loss of coverage letter, etc.
How do I submit a birth certificate and Social Security number for my newborn?
Once you have received this information, you can email it to benefits@cityofomaha.org or call 402-444-5509 if you would prefer to provide the Social Security number over the phone.
How do I get verification of my health insurance?
You can email benefits@cityofomaha.org to request verification of health insurance (existence of coverage, start or end of coverage, dependent coverage, etc) and HR will provide you with a letter.
When do I age off health insurance for Medicare? Is it automatic?
If you are an employee, you may continue on the City’s health insurance as long as you are actively employed, even if you also sign up for Medicare.
If you are a retiree, your last day of City coverage is the last day of the month prior to your birth month when you reach age 65 (ex: birthdate is May 15th, coverage ends April 30th). Medicare starts the first of the month of your birth month (ex: birthdate is May 15th, coverage starts April 30th). Coverage will end automatically.
When do my dependents age off the City plan? Is it automatic?
Dependents age off the plan at the end of the month in which they turn age 26 (ex: birthdate is July 15th, coverage ends July 31st). Coverage will end automatically.
How does my dependent sign up for COBRA?
If health, dental or vision coverage ends, continuation of COBRA coverage is offered. Information will be mailed to you at the time of loss. Our COBRA administrator is HSA Bank and you may contact them at 800-357-6246. COBRA would begin the first of the month following the loss of coverage.
What is FMLA and how do I apply for it?
FMLA (Family Medical Leave Act) provides you with up to twelve weeks of job-protected leave if you have a serious health condition or if you need to care for a family member with a serious health condition. Please contact benefits@cityofomaha.org with questions.
How do I file a claim under my accident insurance or critical illness insurance plan?
Both of these plans are administered by Allstate. You can register and submit claims online at mybenefits.allstate.com. You can contact our Allstate rep, Liz Salkin, at 402-964-5636 with further questions or issues.
How do I file a claim under my hospital indemnity insurance plan?
This plan is administered by Assurity. You can register at myassurity.com and submit claims. You can contact our Assurity rep, Liz Salkin, at 402-964-5636 with further questions or issues.
How do I complete my New Hire Enrollment on my self service dashboard?
You can enroll yourself at employeeportal.cityofomaha.org. Once here, click the Life Event icon where your new hire enrollment event should be waiting. You can also call the Benefits Call Center at 888-350-4209 to complete enrollment over the phone.
How do I get my Form 1095-C?
The City is no longer required to automatically issue Form 1095-C to employees and retirees unless requested. If you would like to request a copy of your 1095-C, please send in your request to benefits@cityofomaha.org or to the City of Omaha Human Resources Department, 1819 Farnam St, Ste 506, Omaha, NE 68183. The form will be mailed to you no later than 30 days after the date your request is received.
