Retirement Information

View Forms

The City is no longer required to automatically issue Form 1095-C to employees and retirees unless requested. If you would like to request a copy of your 1095-C, please send in your request to benefits@cityofomaha.org or to the City of Omaha Human Resources Department, 1819 Farnam St, Ste 506, Omaha, NE 68183. The form will be mailed to you no later than 30 days after the date your request is received.
